How to make a referral

Please complete the registration form prior to your meeting with the therapist

You can arrange a Roundway Centre consultation by completing and submitting the referral form below.

Please note we are not a crisis service, but recommend that if your child needs treatment for a mental health crisis, contact 111  (open 24/7 every day), Text "SHOUT" to 85258 to contact the Shout Crisis Text Line, or text "YM" if you're under 19. If they are under 19, you can also call 0800 1111 to talk to Childline. If there is an immediate threat to life, contact 999.

Information for your visit

Your visit may be face-to-face or through a teleconference including phone or internet e.g. a secure Zoom or Skype consultation.
